Position Description:

The Safety Net Case Manager is a fulltime, non-exempt, position that provides differential response, case management or concrete support services, and/or post reunification or after care case management services to families referred by the Child Welfare System, specifically Child Protective Services (CPS), to maintain the family unit and prevent or reduce the risk of families entering or re-entering the Child Welfare System. This position is primarily responsible for engaging families and connecting them with resources and services to build resilience again risk factors, learn skills to improve family dynamics, alleviate stressors within the family unit, etc. The Safety Net Case Manager works directly with Program Managers and Coordinators to ensure effective interventions for client cases and/or to address obstacles and barriers clients may be facing. Safety Net Case Managers will also provide a range of clerical and administrative duties which include but are not limited to covering the reception area, greeting office visitors, handling incoming communications, managing files, updating paperwork and other documents, perform other general office and clerical duties. Safety Net Case Managers may focus on one area of responsibility, based on community and program needs and contract/funding source requirements.

Duties/Responsibilities:

Direct Services – Provide quality, trauma-informed advocacy to support the following client or program needs:

  • Engage prospective clients in an effort to link them to Safety Net Services, including but not limited to, conducting in-home visits, making contact with clients via face-to-face meetings, phone and mail, maintaining communication with the referring CPS social worker, etc.
  • Engage clients in maintaining services to increase resilience, protective factors, and self-sustainability, including but not limited to, developing and monitoring case plans with families, connecting clients to services within PREVAIL and outside referrals to community resources and supportive services, etc.
  • Regularly follow up with clients to monitor their progress toward goals.
  • Crisis intervention may include providing immediate assistance in times of crisis (e.g., domestic violence, sexual assault, homelessness, mental health emergencies, etc.).
  • Advocacy may include acting as an advocate for clients, ensuring they understand their rights and the services available to them and help clients navigate complex systems and overcome barriers to accessing services.
  • Keep detailed records of client interactions, progress, and services provided.
  • Maintain a network of communication, collaboration, and cooperation between medical treatment facilities, law enforcement agencies, Victim Witness, Child Protective Services, etc.
  • Reinforce policy and procedures with staff and clients; handle challenging client situations; and other related tasks.
  • Participate in and attend regular on-going trainings.
  • Initiate and/or maintain communication with community partners, as needed and directed by Program Coordinator or other leadership members.
  • Use conflict resolution and de-escalation skills as necessary to maintain a healthy and neutral environment.
  • Community Engagement: Represent the agency at internal and external events including, but not limited to, program partnerships, funder/contract reporting meetings, multi-disciplinary team meetings, Child and Family Team (CTF) meetings, etc.; may provide educational presentations to a variety of community-based organizations and/or partners.
Administrative:
  • Execute comprehensive and detailed documentation, and perform other administrative tasks, as necessary, to support both program operations and role responsibilities.
  • Provide administrative support by handling document preparation, data entry, and office organization.
  • Manage office supplies, maintain files and calendars, and oversee the distribution of incoming and outgoing mail.

Education and Experience:
  • Bachelor’s degree with a major in psychology, sociology, or related field
  • In lieu of bachelor’s degree: 60 completed semester units with a major course in psychology, sociology, or related field plus two years of experience providing case management services to children and families. Experience shall be verified as having been performed as a paid or volunteer staff person whose duties required direct supervision.
  • Reliable transportation, valid California Driver’s License, and proof of insurance.
Physical Requirements:
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift 50 pounds at times.
  • Must be able to ascend and descend up to three flights of stairs throughout a typical workday.

Position Description:

The Youth Shelters Program Specialist (Per Diem) works on an as needed basis and provides onsite support to the youth shelter programs. This position is responsible for handling day-to-day operations regarding facility needs, licensing compliance, team needs and shelter client needs. In addition, this role may provide shelter shift coverage with varying hours, requiring trauma-informed intervention services to support the healing of families, individuals, and children that have experienced domestic violence, sexual assault, and the aftermath of such victimizations, including human trafficking and youth homelessness. These services may include but are not limited to crisis intervention; support services, including peer counseling, educational classes, support groups, and case management; and after-care services. Program Specialists work directly with Program Coordinators and other leadership members to ensure effective interventions for client cases and/or to address obstacles and barriers clients may be facing.

Duties/Responsibilities:

Direct Services – Provide quality, trauma-informed advocacy to support the following client or program needs:

  • Role model day-to-day living skills with shelter clients, including but not limited to, meal preparation, laundry, housekeeping, and other appropriate habits; provide supervision to clients in the shelter; ensure appropriate shelter conditions, including but not limited to, cleanliness and supplies; ensure appropriate implementation of shelter procedures and/or any licensing regulations; provide in-person and over-the-phone intervention services to prospective and current clients, such as crisis intervention and peer counseling.
  • Reinforce shelter policy and procedures with staff and clients; handle challenging client situations; and other related tasks.
  • Participate in and attend regular trainings related to licensing regulations.
  • Initiate and/or maintain communication with community partners, as needed and directed by Shelter Manager or Coordinator.
  • Peer counseling: Services include psychoeducation and intervention in a one-to-one setting, to meet client needs; services may occur in-person, via telephone or teleconferencing; etc.
  • Provide communication of care among and not limited to; guardian, school, and Child Protective Services, when applicable, and assist in compliance monitoring and site visits of Community Care Licensing regulations (Youth Shelter).
  • Acts as onsite facility manager when on duty.
  • Participate in staff rotation of backup shift coverages.
  • Community Engagement: Represent the agency at internal and external events including, but not limited to, program partnerships, funder/contract reporting meetings, multi-disciplinary team meetings, etc.; may provide educational presentations to a variety of community-based organizations and/or partners.
  • Supervise client interaction and use conflict resolution and de-escalation skills as necessary to maintain a healthy and neutral environment. Interact with youth and encourage engagement in services that could be beneficial. Connect clients to services within PREVAIL and outside resources as appropriate.
Administrative:

Availability:
  • The Youth Shelters Program Specialist (Per Diem) may be called into work based on the availability schedule provided by the employee. The employee is unrestricted to use their time away from work for their own purpose. Due to the unrestricted nature of “free” time, the employee is not under the control of PREVAIL and will not be paid until actual work is performed.

Position Description
  • The Domestic Violence Program Specialist is a part-time, non-exempt position that provides onsite support to the domestic violence shelter programs. This position is responsible for handling day-to-day operations regarding facility needs, compliance, team needs and shelter client needs. In addition, this role may provide shelter shift coverage with varying hours, requiring trauma-informed intervention services to support the healing of families, individuals, and children that have experienced domestic violence, sexual assault, and the aftermath of such victimizations, including human trafficking and youth homelessness. Additionally, PREVAIL is an at-will employer. Employees, as well as the agency itself, are free to terminate employment status at any time, with or without reason. Although PREVAIL may choose to terminate employment for cause, cause is not required.

PREVAIL is a highly established nonprofit organization funded by a variety of federal, state, and local government agencies, foundation grants and individual supporters. The continuation of programs, and therefore employment positions, may be contingent upon continued receipt of these or alternative funding sources.
